NOTICE: If your application involves data obtained from the Department of Health, Office of Healthcare Statistics, there may be fees. For requests from the Hospital Discharge Data System (HDDS) or Ambulatory Surgical Data System, please use the Tennessee Department of Health Public Records Request Portal (Link Below).

Notice to Applicants filing CON applications for cardiac catheterization services. Beginning September 22, 2026, please use the Tennessee Department of Health's Joint Annual Report to calculate diagnostic equivalent procedure and weights for measuring cardiac catheterization laboratory capacity. The utilization located in the Hospital Joint Annual Report under Schedule D, Item 18.D.
