Anyone looking to apply for home health accreditation through ACHC can set up a Customer Central account at the below website. After that is completed, they will be contacted by a Customer Care team member that can talk to them about the standards, ACHC survey process, timeline for survey, etc.

Community Health Accreditation Partner (CHAP)

CHAP is an independant, non-profit, accrediting body for home and community based healthcare organizations.  As the first accrediting body in home healthcare, CHAP was created as a joint venture between the American Public Health Association and the National League for Nursing in 1965.
