Preferred Provider Organization, or PPO, health plan members don’t have to meet a deductible for in-network, outpatient physical therapy, speech therapy, occupational therapy, and applied behavioral analysis. Premier, Standard and Limited PPO members only pay coinsurance. Copay PPO members only pay a copay.

Cardiac rehabilitation

Health plan Premier, Standard and Limited PPO members pay no deductible or coinsurance for in-network, outpatient cardiac rehabilitation services. Consumer-driven Health Plan, or CDHP, members pay the deductible but no coinsurance. Copay PPO members pay a copay.
