Physical, occupational and speech therapies
Preferred Provider Organization, or PPO, health plan members don’t have to meet a deductible for in-network, outpatient physical therapy, speech therapy and occupational therapy. You only pay coinsurance.
Health plan members can lower their out-of-pocket costs on certain maintenance drugs with a 90-day supply from a 90-day network pharmacy or by mail order.
The maintenance tier allows you to get a 90-day supply of certain drugs from a Retail-90 or mail order pharmacy at a reduced cost. The maintenance drug list includes certain medications for high blood pressure, high cholesterol, coronary artery disease, congestive heart failure, depression, asthma/chronic obstructive pulmonary disease, statins for high cholesterol, depression and diabetes (oral medications, insulins, needles, test strips and lancets). Some osteoporosis medications have been added to the maintenance drug tier list. This does not apply to any osteoporosis medications considered as a specialty medication (some examples include: Forteo, Prolia, Reclast and Evenity).
A list of participating Retail-90 pharmacies can be found at info.caremark.com/stateoftn.
Health plan PPO members pay no deductible or coinsurance for in-network, outpatient cardiac rehabilitation services. Consumer-driven Health Plan, or CDHP, members pay the deductible but no coinsurance.
