2025 Form 1095-C —NOW AVAILABLE

Benefits Administration only produces 1095-C forms for Central State employees and retirees who were offered and/or enrolled in medical insurance for the calendar reporting year. If you are a higher education, local education or local government employee or retiree, contact your agency’s benefits coordinator to inquire about your 1095-C form.
As of March 2, your 2025 Form 1095-C is available in Edison Employee Self Service for you to view and/or print for your records. You can log into Edison at Edison.tn.gov to get your form. If you did not consent to an electronic 1095-C form, the form is still available in Edison self-service, and you can print it for your records. Forms will not be mailed automatically. You may request a mailed paper copy by contacting benefits.administration@tn.gov. In the email subject line include: 1095-C form request. In the email body: include your first and last name, full home address, date of birth, last 4 of your social security number, or your full Edison employee ID. This information is necessary to verify HIPAA information before mailing the form to you.
