TASC is the new vendor for 2026. The 2026 HSA contribution form cannot be submitted until December 2026.

This is the employee payroll deduction form to submit to their agency.  This is an internal form to be used by agencies that do not have their own internal form.
This form is not to be submitted to TASC.
