Get Personalized Care Support

Case Management

Case managers can help coordinate your care, explain treatment options, connect you with resources and help you navigate your health plan.

Your medical carrier may contact you if you could benefit from case management, or you can call Member Services to ask about the program.

BlueCross BlueShield Members

• Call 800.558.6213.
• Use the CareTN app to chat with a nurse or other health professional for support with long-term health conditions. Log into your BCBS account to access the program.
• Search for “Care TN” if you aren’t automatically directed to the Care TN One-on-One Health Support page.

Cigna Members

Manage Your Chronic Condition

Sharecare Health Coaching

One-on-one coaching is available for members living with chronic health conditions, including:

• Diabetes
• High blood pressure
• Asthma
• Chronic kidney disease
• Heart failure
• Chronic obstructive pulmonary disease, or COPD
• Coronary artery disease

Your coach can help you set goals, close care gaps and better manage your condition.

Expert Medical Opinions

Expert medical opinion services are available at no additional cost to help confirm diagnoses, review treatment options and provide guidance for complex or rare conditions.

Centers of Excellence

Carrum Health

Members aged 18 and older enrolled in a Partners for Health medical plan have access to Carrum Health's Centers of Excellence for specialized surgical care and comprehensive cancer care.

Eligible care through Carrum Health may be covered by your medical plan at little or no cost to you.

Transplant Benefits

• Prior authorization is required for all transplant procedures.
• Travel expenses may be covered for qualifying trips of 60 miles or more each way.
• Using your plan's designated transplant facilities may reduce or eliminate your out-of-pocket costs.
  • Consumer-driven health plan members must meet their deductible before costs are waived.
  • Usual member costs apply if another provider or facility is chosen when there is a cost-saving option available.
