TennCare is aware that DentaQuest experienced a data breach and is notifying affected TennCare members. Members who receive a notification letter, or who have questions about the incident, can contact the dedicated DentaQuest help line at (844)959-7163 or visit DentaQuest for additional information.

TennCare is notifying members about a privacy incident involving member mail that may have affected at least 2,300 people. We will update this webpage if we learn about a change to the type of information or number of people impacted.

What Happened

Beginning in December 2025, some member address changes were incorrectly updated. As a result, mail intended for certain TennCare members may have been delivered to the wrong address. We identified the issue on May 28, 2026.

The incorrect addresses were also provided to a member’s health plan, dental plan, and pharmacy plan. Therefore, this issue may have affected any mail from the health plans, Pharmacy Benefits Manager, and Dental Benefits Manager.

This issue only affected members who receive notices by mail.

What Information was Involved

For some people, the information in those letters may have included:

  • Household members and demographic information
  • TennCare identification numbers
  • Coverage status and type
  • Financial information such as income
  • Claims information found on mailed EOBs
  • Health insurance ID and other welcome materials
  • Last four (4) digits of Social Security Number

What TennCare is Doing

We take the privacy and security of member information seriously. Upon learning of this issue, the agency:

  • Immediately began an internal review to assess the incident’s full scope;
  • Engaged the Privacy Office to coordinate investigation and appropriate notifications;
  • Implemented corrective steps to prevent similar incidents from occurring in the future;
  • Began preparing individual notifications to all people whose information may have been affected;
  • Provided media notice in accordance with federal requirements under the Health Information Portability and Accountability Act (HIPAA); and
  • Ensured no negative impact to eligibility.

TennCare is mailing notification letters to members who may have been impacted.

What You Can Do

At this time, there is no evidence that any information has been misused. However, anyone affected should look for mail or communications that appear inaccurate or unexpected. Impacted individuals will receive credit monitoring information by mail.
