Health Promotion and Prevention
Evidence-based programs for health promotion and disease prevention offer proven ways to encourage and support healthy aging among older adults. Chronic disease self-management, physical activity, nutrition, and falls prevention are key areas for healthy aging, all supported by evidence-based programs.
These programs are based on reliable studies of the effects of specific approaches or model programs. They demonstrate consistently positive changes in important health-related measures and in the way participants improve and have tools in place to maintain program access, quality, and efficiency in many different settings.
There is established criteria for determining if programs are evidence-based, and approved programs are eligible for funding through the Older American’s Act Title III-D or other discretionary funding. There are many approved evidence-based programs offered in Tennessee, a few are highlighted here.
Stay Active and Independent for Life (SAIL)
The SAIL program is a strength, balance and fitness program for adults 65 and older. SAIL was started in Tennessee through a partnership with the East Tennessee Area Agency on Aging and Disability and the Knox County Health Department.
Chronic Disease Self-Management Education (CDSME)
Chronic Disease Self-Management Education (CDSME) programs provide older adults and adults with disabilities with education and tools to help them better manage chronic conditions such as diabetes, heart disease, arthritis, chronic pain, and depression.
