These are the required forms to obtain a mine license and to apply for the certificate of competency .
Persons seeking to become certified as underground coal mine foreman must complete the necessary paperwork and have worked in underground coal mines for a minimum of five years. |

Persons seeking to become certified foreman in underground metal/nonmetal mines must complete the forms and have a minimum of three years experience. Persons seeking to become certified foreman in underground metal/nonmetal mines must complete the forms and have a minimum of three years experience. |

Persons seeking to become certified as mine foreman in surface metal/nonmetal must fill out the paperwork and have a minimum of one year of surface metal/nonmetal experience. |

Persons seeking to be certified as surface coal mine foreman must fill out forms and have a minimum of two years of surface coal experience. |
||
Persons seeking to become certified as prep plant foreman must complete forms and have a minimum of two years prep plant experience. |

Completed application needed for license to operate a mine. |
