Request an Owner Operator Report Record
A Financial Responsibility Affidavit is a notarized statement issued by the Department of Safety and Homeland Security upon request from an individual or his representative stating whether a person involved in a crash has filed an Owner/Operator Report indicating liability insurance for a specific crash.
If this notarized statement is requested, then there is a charge of $5 payable by cashier's check or money order made out to the Department of Safety and Homeland Security. Please allow two weeks from mailing date for return of affidavit.

- Driver license number of person involved in crash.
- Date of accident
- County of accident.

Removal of STOP on Vehicle Registration
If a STOP has been placed against your vehicle file for failure to provide evidence of financial responsibility under 55-12-139 before you can renew your registration, you will need to reinstate you driving privileges if suspended by meeting all requirements necessary for reinstatement. If the STOP was because of a dismissal under T.C.A. 55-12-140 for submitting evidence of financial responsibility prior to the court's disposition, then you will need to submit proof of Financial Responsibility again before renewing your registration.

You will need to contact a reinstatement agent at 1-866-903-7357 to verify receipt of the proof and then allow three (3) days for the STOP to be removed from the vehicle.
Submit an Accident Claim against an Adverse Party
Pursuant to TCA 55-12-105, an individual or business acting on behalf of the claimant (insured party), may request the revocation of an adverse party’s driving privileges by filing an Accident Claim with the Tennessee Department of Safety and Homeland Security.
Accident Claims must be received by the Department within 2 years 10 months of the date of the accident.
Individuals and Businesses can file an Accident Claim via the following methods:

The documents below MUST be provided with Accident Claims:
- A Letter of Request for Revocation which must include:
- Adverse Party (Person at fault) Full Name, Driver License Number and Date of Birth.
- Adverse Party (Person at Fault) Vehicle Owner Full Name and Driver License Number
- Claimant (Insured Person) Full Name
- Date of Crash (must be within 2 years and 10 months)
- Amount of Damages
- A Notarized Affidavit of Damages
- When the accident damages are above $1500.00, but the crash report lists damages under the threshold.
- When the accident damages of state or local government property are above $400.
- Must include a sworn statement of the amount of damages.
- A Full Crash Report – This can be obtained from the Police Department of the Investigating Officer.
Note: Please do not submit photographs with the Accident Claim filing, these are not required.
Unsatisfied Judgements must be mailed and cannot be accepted through the Tennessee Department of Safety e-Services Portal currently.
