Owner Address Changes

To change the address of a current owner please submit the following form within 30 days of change:

• Change of Owner Mailing Address Amended Notification (CN-1383)

Changes in Status and Contacts

You must submit the following form within 30 days of change.  The entire form does not have to be submitted, page 2 and 3 must always be included as well as the pages with the relevant change.

• Notification for Underground Storage Tanks (CN-1260)

Changes including but are not limited to the following:

• Compartment status such as “Currently in Use” to “Temporarily Out of Service”
• Upgrading or replacement of tanks or lines
• Service of tank 
• Contact person information
• Release detection 
• Owner Name Change
• Facility Name Change
