Installation of New Tanks
The following form should be received by the division 15 days prior to installation.
Payment of annual tank fees $125.00 per tank compartment is also due at this time. This does not complete registration!
The tanks are listed as Temporarily Out of Service until the Division receives the following form:
This form should be received no later than 15 days prior to completed installation.
Note: The form CN-1260 can be used as the pre-installation form in you are able to fill it out in its entirety and submit it 15 days prior to installation.
