Archived Record Request

The Department of Correction frequently receives requests for historical records on offenders.  Requests for records can now be submitted through our new online web portal.

SUBMIT A REQUEST

Requests can also be submitted in writing to:

You will need to include the following information with your request:

  1. Your contact information - name, mailing address, phone number and an email address, if available.
  2. The reason for your request.
  3. The full name of the offender, Tennessee offender identification number (TOMIS #), date of birth, and social security number.
  4. A detailed description of the information you are seeking from the record.

If an archive search is required, appropriate charges will apply.  Do not send a payment with your original request.  You will be notified of any charges when the request is completed.
