The Department’s Office of Licensure also can assist you in locating a licensed mental health and/or substance abuse provider in Tennessee. Additionally, you can contact the office nearest you for general information, file a formal complaint, and inquire about becoming a licensed service provider.

Offices are open weekdays, Monday - Friday, 8 a.m. - 4:30 p.m., excluding State holidays.
