Form C-31What is It and Why is it Needed?
When a worker is injured on the job, insurance companies need access to medical records to process the claim. Although Tennessee workers’ compensation law allows these records to be shared without a signed release, many medical providers still require one due to privacy concerns.
This can lead to delays, confusion, and frustration for everyone involved.
The Medical Waiver and Consent Letter (Form C-31) helps resolve this issue. It gives medical providers the assurance they need to release injury-related records promptly and appropriately, while keeping the claims process moving forward without unnecessary delays.
There Are Two Versions. Which One Do You Need?
To meet the needs of different types of claims, there are currently two versions of the Medical Waiver and Consent Letter:
- Form C-31A: Use this form if the injury occurred before July 1, 2014, under the old workers’ compensation law.
- Form C-31B: Use this form if the injury occurred on or after July 1, 2014, under the current workers’ compensation law.

How Does the Process Work?
Typically, this form comes into play when an insurance company or employer representative needs medical documentation to evaluate or process a claim. In these situations, the C-31 form may be requested by either party to facilitate the release of medical records from the treating provider. Once completed and signed by an injured worker, the form is sent to the medical provider along with a request for records. The provider then sends the relevant medical documentation to the requesting party.
What to Expect: Timeline and Tips
Typical Response Time: Under Tennessee workers’ compensation regulations, medical providers must release the medical records of any employee treated for a work-related injury within 30 days after admission or treatment. Delays in receiving medical records can slow down the claims process, potentially affecting benefits and treatment approvals.
