Who is required to submit an annual report?
Each agency identified by the Title VI program must submit one report for the applicable reporting cycle. The designated Title VI coordinator is responsible for coordinating and submitting the report.
What reporting period does the form cover?
The Fiscal Year 2025-2026 form covers activities from July 1, 2025, through June 30, 2026. Use the period shown on the current form and instructions.
When is the report due?
The completed report and all required supporting materials are due by October 1, 2026, at 4:30 CT. Contact Michael McVey and Melanie Koewler promptly if circumstances may affect timely submission.
Which form should I use?
Use only the current form posted on the “Reporting Form and Required Materials” page. Confirm that the reporting year and version date match the current cycle.
What documents are required?
Submit the completed annual reporting form and each supporting document identified in the annual instructions and checklist. Requirements may change by reporting cycle.
How will I know the report was received?
Title VI Coordinators will receive a confirmation email from DOHR’s Legal Compliance team confirming their submission receipt.
Does confirmation mean the report is approved?
No. Confirmation means the files were received. Program staff may contact you with questions or request revisions during review.
How do I correct or replace a submission?
