Annual Reporting Instructions
Review the requirements below before beginning your annual Title VI report. The designated coordinator is responsible for ensuring the report is complete, accurate, approved as required, and submitted by the deadline.
Reporting Period and Deadline
The FY 2025-2026 report covers activities from July 1, 2025, through June 30, 2026. Completed reports and supporting materials must be submitted by October 1, 2026. Submissions received after the deadline may be identified as late and may require additional follow-up from the Title VI program.
Who Must Submit
Each agency required to participate in the annual Title VI reporting process must submit one complete report. The designated Title VI coordinator should coordinate with appropriate program areas, leadership, legal counsel, or other subject-matter experts as needed to compile and verify the information.
Coordinator Responsibilities
- Use the current approved reporting form.
- Collect complete and accurate information for the reporting period.
- Confirm that required reviews, approvals, signatures, and certifications are complete.
- Ensure all required supporting materials are included.
- Submit the report through the approved SharePoint process by the deadline.
How to Submit
- Download or open the Title VI Compliance Report and Implementation Plan Submission Guidelines.
- Complete every applicable section. Enter “Not applicable” only when permitted by the instructions; do not leave required fields blank.
- Save the completed form using the required naming convention.
- Prepare and name each supporting document.
- Select “Submit Annual Title VI Report” to open the SharePoint submission page.
- Upload the completed form and all required supporting documents.
Required Materials
Submit the following materials unless the annual instructions state otherwise:
- Completed FY 2025-2026 Title VI Annual Reporting Form.
- Appendices.
Any additional documentation specifically requested by the Title VI program.
File Requirements
- Accepted file types: PDF and XLSX.
- File names: “[Agency Name] FY 2025–2026 Title VI Compliance Report and Implementation Plan”
- Verify that files open correctly before upload.
- Do not password-protect or encrypt files unless the program provides specific instructions. Title VI Compliance Report and Implementation Plans are confidential between DOHR’s Legal Compliance and the agency.
What Happens After Submission
The Title VI program will review submitted materials for completeness and may contact the coordinator with questions or a request for clarification or revision. Title VI Coordinators will receive a confirmation email from DOHR’s Legal Compliance team within one business day of submission.
