The Civil Rights Act of 1964Pursuant to Tennessee Code Annotated § 4-3-1706, it is discriminatory for a state agency receiving federal funds, which makes it subject to Title VI of the Civil Rights Act of 1964 (42 U.S.C. § 2000d et seq.), or any person receiving federal funds from a state agency, to exclude a person from participation in, deny benefits to a person, or subject a person to discrimination in any program or activity receiving such funds, on the basis of race, color, or national origin. Title VI Complaint Form
If you feel you have been treated unfairly by a department-funded community behavioral health provider, based on the protections offered through Title VI of the US Civil Rights Act, you can file a formal complaint. Follow the instructions on the Title VI Complaint Form MH-5479.
