DT-0330 Part II Form (Advertisement Specific Qualifications)

Part II of the DT-0330 form is advertisement specific and must be submitted only  firms who have been short-listed by TDOT. TDOT will notify the appropriate firms when submission is required.

Part II of the DT-0330 form must be submitted to the TDOT offcial who requested it.  Any questions regarding the form should be directed to TDOT.PSAdvertisements@tn.gov.
