UT Extension: Living Well with Chronic Conditions– The Tennessee Department of Health in partnership with the University of Tennessee-Extension offers the self-management workshops and leader trainings across the state.

Aging and Disability Resource Connection

Commission on Aging and Disability website provides a list of agencies that plan and provide programs and services for older Tennesseans as well as those with disabilities at the Tennessee Area Agencies on Aging and Disability. Also available is the Aging and Disability Community Resource Guide addressing quality of life issues and changes that accompany aging.
