County Health Councils conduct the County Health Improvement Process every 3 years, including both the County Health Assessment (CHA) and the Community Health Improvement Plan (CHIP). The interactive tool below showcases previous and current County Health Assessment Cohort counties and their selected health priority areas. For more information on which counties selected a certain health priority area, please click on the tabs across the top.
