Available to: Person with spouse, parent, sibling or child who served and died in the line of duty in the U.S. armed forces or its allies, or died as a result of injuries sustained in such service.

Documentation Required: DD Form 1300 Report of Casualty.

Cost: None for first plate. Additional plates - $26.50 annual fee (initial fee is $29.80)
