Immunization Certificate Validation Tool (ICVT)
Through the Tennessee Immunization Information System (TennIIS), authorized users can access the Immunization Certificate Validation Tool (ICVT) to print out validated Official Immunization Certificates for children. The validation tool produces an Official TN Immunization Certificate that does not require the signature of a healthcare provider with complete records, validation failure reports for children with incomplete records, and temporary certificates for children who are as up-to-date as possible. It compares a child’s record in the state immunization registry against TN immunization requirements for pre-school or school attendance.
TN Immunization Certificate Sample
The ICVT can be used each time an Official TN Immunization Certificate is required:
- At child care enrollment or when a child is old enough (18 months old, or older) to have completed required vaccines for childcare
- At kindergarten enrollment
- When a child enrolls in a school in TN for the first time, but is enrolling in a grade other than kindergarten
- When all students enter 7th grade (they must have a Tdap booster and a 2nd dose of chickenpox vaccine, if they haven’t had chickenpox)
If a record does not meet the state requirements, it will produce a failure report showing which required vaccines are still needed. In addition, the tool will produce a temporary certificate for the child who has not completed the required immunizations, but is as up-to-date as possible. The certificate expires 1 month after the date that the next catch-up immunization is due. It does not produce certificates for children with medical or religious exemptions to immunization.
All authorized users of TennIIS may use the tool and print out validated Official Immunization Certificates for children with complete records, validation failure reports for children with incomplete records, and temporary certificates for children who are as up-to-date as possible.
