On Track TN: First Episode Psychosis Initiative

On Track TN is designed to provide early intervention services for youth and young adults ages 15 to 30 who have experienced first-episode psychosis.  This comprehensive intervention model is a team of mental health professionals and support services, focusing on helping people work toward recovery and meeting personal goals.  The program includes the following components:

  • Individual and Group Psychotherapy
  • Supported Employment and Education
  • Family Education and Support
  • Peer Support
  • Psychopharmacology
  • Care Coordination and Management

On Track TN services are available in select counties through the following providers:

Clinical High Risk for Psychosis

The Clinical High Risk for Psychosis (CHR-P) program utilizes a stepped-care model to assist youth and young adults ages 12 to 25 who are at clinical risk for developing psychosis for the purpose of improving access, quality, and utilization of services and supports for youth, young adults, and their families.  The CHR-P service model includes the following components:

  • Care Coordination
  • Family Peer Support
  • Young Adult Peer Support
  • Supported Employment and Education Services
  • Medication Management
  • Therapy Services

Statewide First Episode Psychosis and Youth & Young Adult Trainer

The Statewide First Episode Psychosis (FEP) and Youth & Young Adult Trainer provides training, technical assistance, and consultation to youth and young adult behavioral health programs across Tennessee. This role supports providers through workforce development, implementation guidance, and coordination with national experts to ensure the delivery of evidence-based practices. Specializing in early psychosis, the trainer provides fidelity oversight and consultation to Tennessee's First Episode Psychosis (OnTrackTN) and Clinical High Risk for Psychosis (CHR-P) programs. The trainer also partners with behavioral health organizations, schools, healthcare systems, and community agencies to build capacity and increase awareness of youth and young adult mental health needs statewide.
