For NCD schools, fill out the following forms and submit at link at bottom:

TN_SAA Program Submission List for all programs

Submit all forms through the Catalog Application Submission link

For Vocational Flight Schools, fill out the following forms and submit at link at bottom:

VA form 22-10288a Non-IHL Program Submission Lists

NOTE: Only complete the Vocational Flight School- Program Submission List Pages 1 and 3, 4, & 5

For Apprenticeship & OJT programs, fill out the following forms and submit at link at bottom:

NOTE: only complete the Apprenticeship, On-the-Job Training, or Multi-state Registered Apprenticeship- Training Program Submission List

Only complete a Catalog Modification IF there are any changes to the program(s), catalog, or other items that would change the student certifications. This is NOT used for a 24-month reapproval.

All facilities submit Catalog Modifications through the Catalog Modification Submission Link.
