District Adoption Compliance Forms

Pursuant to T.C.A § 49-6-2207 (e), upon the completion of local adoption processes, directors of schools are required to submit documentation of the district’s High Quality Instructional Materials (HQIMs) adoption decisions to the department of education.

The ED-2153 form signed by the local board and the ED-5099 which provides a list of which HQIMs were adopted by the district must be submitted to the department of education.

Forms Submitted for State Compliance: ED-2153, ED-5099

Documentation of the local committee is noted on ED-2153 which remains filed at the local level for the duration of the adoption.

Form Retained at the District Level: ED-2154

Completed documents for the current science materials adoption cycle should be sent in one email as attachments to Academics.Instruction@tn.gov. The deadline to submit science textbook adoption forms ED-2153 and ED-5099 is June 30, 2025.
