District Adoption Compliance Forms

Pursuant to T.C.A § 49-6-2207 (e), upon the completion of local adoption processes, directors of schools are required to submit documentation of the district’s High Quality Instructional Materials (HQIMs) adoption decisions to the department of education.

Both the ED-2153 form signed by the local board and the ED-5099 which provides a list of which HQIMs were adopted by the district must be submitted to the department of education. These forms need to be electronically submitted utilizing the links below.

Note: If a district is not re-selecting ELA materials to be utilized for academic years 2026-27 and 2027-28, revised forms do not need to be submitted.

Documentation of the local committee is noted on ED-2154 which remains filed at the local level for the duration of the adoption.

Completed documents for the current Schedule E adoption cycle (CTE and PE/Health Education) should be submitted using the Formstack links provided.  The deadline to submit the Section E textbook adoption forms ED-2153 and ED-5099 is June 30, 2026.
