About CDC 2304

In 2023, the Tennessee Department of Health (TDH) was awarded a 5-year Centers for Disease Control and Prevention (CDC) 23-0004 Cooperative Agreement entitled “The National Cardiovascular Health Program.”

To support 2304 goals and objectives, TDH utilizes funding to conduct activities in the following areas: 

  • Track and Monitor Clinical and Social Services and Support Needs Measures Shown to Improve Health and Wellness, Health Care Quality, and Identify Patients at Highest Risk of Cardiovascular Disease (CVD) with a Focus on Hypertension and High Cholesterol.
  • Implement Team-Based Care to Prevent and Reduce CVD Risk with a Focus on Hypertension and High Cholesterol Prevention, Detection, Control, and Management through the Mitigation of Social Support Barriers to Improve Outcomes.
  • Link Community Resources and Clinical Services that Support Bidirectional Referrals, Self-Management, and Lifestyle Change to Address Social Determinants of Health (SDoH) that Put the Priority Populations at Increased Risk for Cardiovascular Disease with a Focus on Hypertension and High Cholesterol.
  • Increase reporting and tracking of clinical data to improve identification, management, and treatment of patients with high blood pressure and cholesterol. 
  • Encourage providers to follow evidence-based guidelines and policies for team-based care of patients with high blood pressure and cholesterol. 
  • Increase community-clinical links that support systematic referrals, self-management, and lifestyle change for patients with high blood pressure and high blood cholesterol. 
  • Encourage patients to continue taking their medicine for high blood pressure and cholesterol. 
  • Assist patients with high blood pressure and cholesterol manage their conditions. 
  • Encourage more patients with high blood pressure and high blood cholesterol to join proven lifestyle change programs.
