Façade Grant Closeout Report

All Façade grantees must submit annual reports and a final report upon project completion (the final report shall take the place of the regularly scheduled annual report for the year in which the project is completed).  The following documents must be completed and accompany the close-out report: 

Applicant/Recipient Disclosure/Update Report

Closeout Public Hearing Sign-in Sheet and Public Announcement

Minutes from the Closeout Public Hearing

Final Wage Compliance Report (as applicable)

Before and After pictures of all buildings impacted by the project.
