Reciprocity and Reinstatement

Verification of License

This is completed through your online CORE account. This request must be completed and all fees paid before the Board’s certification to another state. There is a fee of $60 per license certification. If the requester is licensed as both a Funeral Director and Embalmer, the Board must receive payment for verification of both licenses ($120).

Manager Change

This is completed through your online CORE account. The fee associated with this application is $60.

 

Establishment Authorization for Cremation Request

Complete the “Establishment Authorization for Cremation Request Form” through your online CORE account.  The fee for the establishment authorization for the cremation form application is $100 per submission. Two copies of the form must be submitted for review.

 

Name Change

1. “Name Change” Application (CORE)

2. Required Information: Must have General Price List, Casket Price List, Outer Burial Container Price List, and Statement of Funeral Goods and Services Selected with the new establishment name.

3. Fee, $260

 

Ownership Change

1. “Ownership Change” Application (CORE)

2. Required Information

a. Letter from Zoning Authority stating that the property is specifically zoned for a funeral establishment or crematory. Stating that the property is commercially zoned or is zoned B-1 or other codes will not be accepted.

b. Letter from the appropriate authority on official stationery stating the establishment has adequate parking space available. If there are no parking requirements, the letter must state that there are no parking requirements.

c. Letter from the appropriate authority on official stationery verifying that the establishment is connected to the sewer system.

i. If the establishment is on a septic tank system, attach a letter from the local county health department indicating that the septic system is in proper working order and for the intended use.

d. If the establishment does not have a preparation/embalming room, submit a letter from the licensed funeral establishment manager, on the letterhead and with a signature, stating the location where preparation of dead human remains will be performed.

e. Copy of the establishment license of the crematory that will be used.

i. If the establishment does not have a crematory onsite, a copy of the establishment’s most recent inspection performing the cremations will be needed.

ii. If the establishment does have a crematory onsite, the State of Tennessee Air Pollution Control Board Department of Environment and Conservation permit is needed. The name on the permit needs to be the exact name of the establishment.  

f. Must have General Price List, Casket Price List, Outer Burial Container Price List, and Statement of Funeral Goods and Services Selected with new establishment name and address.

g. If Corporation/LLC:

i. List of officers, titles, addresses, and registered agent

ii. Copy of the organization from the Secretary of State

h. List of all employees with:

i. Residential addresses and personal phone numbers

ii. Indication of full-time or part-time employment

iii. State whether the employee is licensed or unlicensed. If the employee is a licensed Funeral Director and/or Embalmer, state the license number(s). If the employee is a preneed sales agent, provide the preneed sales agent registration number.
