Tennessee Women's Heart Health Program

TWHHP provides heart disease risk factor screening, blood pressure control education, referrals for medical evaluation, and lifestyle programs to low-income, uninsured women in the Tennessee.

About the Tennessee Women's Heart Health Program (TWHHP)

The Tennessee Women’s Heart Health Program (TWHHP) offers heart disease risk screenings, blood pressure education, medical referrals, and lifestyle support for women. Extending the services of the Tennessee Breast and Cervical Screening Program, TWHHP helps participants take an active role in their health and make lasting changes to reduce their risk of heart disease.

Who is Eligible?

You may qualify for TWHHP if you:

  • Are a female resident of Tennessee
  • Are between 35 and 64 years old
  • Are at risk for or living with cardiovascular disease (CVD)
  • Are currently enrolled in the Tennessee Breast and Cervical Cancer Screening Program (TBCSP)
  • Are uninsured or underinsured
  • Have a household income at or below 250% of the federal poverty level

What Services Are Provided?

Qualifying participants can receive free services, including:

  • Blood pressure testing
  • Cholesterol testing
  • Blood sugar (glucose or A1c) testing
  • Referrals to health programs (e.g., health coaching, blood pressure self-monitoring, chronic disease management)
  • Medical referrals and follow-up as needed
