Reportable Diseases

Communicable diseases can easily spread from person to person. With prompt reporting public health officials can locate and treat exposed persons, identify and contain outbreaks, and interrupt disease transmission.

2026 Reporting Guidance

Certain diseases and events are communicable or dangerous to the public and must reported to the Tennessee Department of Health by all healthcare providers (inpatient or outpatient), laboratories, and other persons knowing of or suspecting a case in accordance with the provision of the statutes and regulations governing the control of communicable diseases in Tennessee (T.C.A. §68 Rule 1200-14-01-.02). 

Department Rules for Reportable Diseases, Chapter 1200-14-01-.02, Excerpted:

(1) All healthcare providers and other persons knowing of or suspecting a case, culture, or specimen of a reportable disease or event shall report that occurrence to the Department of Health in the time and manner set forth by the Commissioner in the List. The Commissioner shall re-evaluate, update, and post the List at least annually and from time to time as appropriate.

(2) The Commissioner shall post the annual update on or before November 15th of each year and this new List shall become effective starting January 1st of the following year. If the Commissioner posts an updated List more frequently than on an annual basis, then the updated List will become effective on the date stated in the List. The List shall be available online at the Department of Health’s web page and in print.

Reporting  Information


Healthcare Providers

Healthcare reporting requirements apply to all providers located within Tennessee, as well as providers whose patients reside in Tennessee.

Providers must report cases of all diseases and conditions listed through one of these methods:

• Mail or fax a completed PH-1600 form to your local health department or fax to the state health office at (615) 741-3857

• Send automatically via electronic case reporting (eCR). See this TDH webpage for more information on eCR, register at the Trader Partner Registration website, or contact MU.Health@tn.gov for assistance.

• Submit online via NBS. NBS is TDH's reportable disease system. To request an NBS account for reporting Complete this user survey to request an NBS account for reporting

• Blood lead levels can be sent via fax ( (615) 741-3857), entered online, or reported using the instructions at this link


Laboratory Reporting

Reporting requirements apply to all laboratories located within Tennessee, as well as laboratories outside of Tennessee that test residents of Tennessee, including laboratories located within healthcare facilities. Healthcare providers and laboratories in the same healthcare facility both have a duty to report. The type of organisms and analytes laboratories must report to TDH for 2026 are indicated, and there are several ways laboratories can report results to TDH.

Alphabetical listing of reportable diseases and events including description, reporting requirements, laboratory testing algorithms, case definitions, investigation protocols, and report forms. 

As of July 1, 2025 the TN State Legislature is requiring reports of Alpha-Gal Syndrome.  This will be added to the Reportable Diseases Lists during the normal revision cycle and be available for January 1, 2026.  In the interim, please follow your typical reporting mechanisms and include alpha-gal syndrome in your data feeds or manual reports.  Please see CDC’s guidance on the condition for more information as we work to develop TN-specific guidance on reporting https://www.cdc.gov/alpha-gal-syndrome/about/index.html.
