Program of All-Inclusive Care for the Elderly

The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive medical and social services to certain frail, community-dwelling elderly individuals, most of whom are dually eligible for Medicare and Medicaid benefits. An interdisciplinary team of health professionals provides PACE participants with coordinated care.

For most participants, the comprehensive service package enables them to remain in the community rather than receive care in a nursing home.

  • You must be 55 years of age or older;
  • You must meet nursing facility level of care;
  • You must live in Hamilton County; and
  • You must be able to live in a community setting without jeopardizing your health or safety.

An Intake Coordinator will assist you in the application 

PACE services are determined on an individual basis, and include, but are not limited to:

  • Primary Medical Care
  • Adult Day Center
  • Medical specialists including, but not limited to: Dental, Vision, Hearing, Foot care, Psychiatry/Counseling
  • Prescription medications
  • Physical, Occupation, and Speech Therapy
  • Medical equipment
  • Social Work
  • Spiritual Care
  • Transportation
  • In-home personal care (bathing and grooming)
  • Referral to out-patient medical specialists
  • Inpatient services as needed
  • Nurses and Medical Providers are on call 24 hours a day, 365 days a year
