Countryside Healthcare And Rehabiliation Admissions Suspension Lifted

NASHVILLE – The Tennessee Department of Health announces that the suspension of admissions at Countryside Healthcare and Rehabilitation nursing home has been lifted effective September 24, 2009. The facility is a 162-bed licensed nursing home located at 3051 Buffalo Road in Lawrenceburg.

The Department confirmed the facility has returned to substantial compliance for state licensing purposes. The federal conditions of participation that led to the immediate jeopardy citations have been corrected as stated in the facility's allegations of compliance, which was approved by federal and state authorities.

On September 9, the nursing home was ordered not to admit any new residents based on conditions found during a complaint investigation and annual survey conducted August 24 through September 1. During the inspection, surveyors found violations of the following standards: administration; admissions, discharges and transfers; performance improvement; infection control; nursing services; pharmaceutical services and food and dietetic services.
