Collierville Health and Rehabilitation Center Admissions Suspension Lifted

NASHVILLE - Tennessee Department of Health announces that the suspension of admissions at Collierville Health & Rehabilitation Center nursing home has been lifted effective March 26, 2008. The nursing home is a 114-bed licensed nursing home located at 490 West Poplar in Collierville. The Department confirmed the facility has returned to substantial compliance for state licensing purposes, and the deficient practices and conditions have been corrected as stated in the facility’s plan of correction, which was approved by federal and state authorities.

On November 29, the nursing home was ordered not to admit any new residents based on conditions found during complaint investigations and an extended survey conducted October 29 through November 6. During the investigation, surveyors found violations of the following standards: administration; nursing services; staffing; pharmacy services; medication administration; resident protection; building construction; and quality of care.
