Boulevard Terrace Rehabilitation And Nursing Center Admissions Suspension Lifted

NASHVILLE – Tennessee Department of Health announces that the suspension of admissions at Boulevard Terrace Rehabilitation and Nursing Center has been lifted. The facility is a 100-bed licensed nursing home located at 1530 Middle Tennessee Boulevard in Murfreesboro. The Department confirmed the facility has returned to substantial compliance for state licensing purposes, and the deficient practices and conditions have been corrected as stated in the facility’s plan of correction, which was approved by federal and state authorities.

On September 29, the nursing home was ordered not to admit any new residents based on conditions found during a complaint investigation and annual survey conducted September 22 through September 24. During the survey, surveyors found violations of the following standards: administration and nursing services.
