This application is for an Initial Participant, which would include any of the following:

Manager
Second (Cornerman)
Matchmaker
 

Requirements

Licensing Requirements

1. Be at least eighteen (18) years of age.
2. Color Photo (headshot)
3. Photocopy of Driver’s License

 

Application & Fees

Complete the “Initial Participant Application” online at core.tn.gov. The fees are as follows:

- Manager: $150
- Second (Cornerman): $75
- Matchmaker: $150
