Tennessee State Board of AccountancyThis form is for CPA license applicants to document work experience required for licensure. It is required to be signed and dated by a licensed CPA in Tennessee or another U.S. jurisdiction. This form should be uploaded to an applicant’s online application.
Authorization for Transfer of Examination Information (PDF)
This form is for CPA license applicants who have successfully completed the Uniform CPA Examination in a state other than Tennessee and were never licensed in that state but wish to have those scores transferred to Tennessee.
Applicants should request that the state board responsible for completing this form email the completed form to accountancy.board@tn.gov.
Firm Ownership and Peer Review Information (PDF):
This form is for an applicant for a firm permit to document firm ownership, CPA employee, and peer review information. It is required to be signed and dated by the firm's residdent manager. This form should be uploaded to an applicant's online application.
Firm Application Experience Affidavit (PDF):
This form is for an applicant for a firm permit to document compliance with experience requirements for individuals responsible for supervising attest services or signing the accountant’s report on the financial statements on behalf of the firm, or to note exemption from the experience requirement. This form is required to be completed for all firm permit applications, regardless of whether the firm is providing attest services or not.
This form is for CPE sponsors who would like to apply to deliver the Tennessee State-Specific Ethics CPE course required by Board rule 0020-05-.03(b).
