Notice of Review

Applications will be heard December 11, 2019 at the Tennessee Health Services and Development Agency meeting. 

Applications noted as Consent Calendar will be heard December 11, 2019.

This is to provide official notification that the Certificate of Need applications listed below have begun the review cycle effective October 1, 2019.  Agencies reviewing these applications are the Tennessee Department of Health and the Tennessee Department of Mental Health and Substance Abuse Services.

 

CN1907-023  BHG XXII, LLC dba BHG Murfreesboro Treatment Center  

     630 Broadmor Boulevard

     Murfreesboro (Rutherford County)

     For the establishment of a non-residential substitution based treatment center for opiate addiction to be located at 630 Broadmor Boulevard, Murfreesboro, (Rutherford County).  The applicant is owned by VCPHCS, LP which is a wholly owned subsidiary of TVG-BHG Intermediate III, LLC dba Behavioral Health Group, LLC.. The estimated project cost is $845,413.

     For the establishment of an ambulatory surgical treatment center limited to the provision of pain management services, located at 2016 Greystone Square, Jackson (Madison County).  The applicant is owned by Midsouth Interventional Pain Institute, LLC.  The estimated project cost is $1,006,782.

     For the establishment of a single specialty ambulatory surgical treatment center (ASTC) limited to oral and maxillofacial surgery and dentistry with 3 operating rooms and no procedure rooms located at 500 28th Avenue North, Nashville (Davidson County).  The applicant is owned by Adam S. Pitts., M.D.  The estimated project cost is $4,559,044.

     For the addition of 5 Medicare certified skilled nursing beds to The Village at Germantown's existing 50 Medicare certified skilled nursing facility (SNF) bed nursing home.  These beds are subject to the 2019-2020 125 nursing home bed pool.  The applicant is owned by The Village of Germantown, Inc.  The estimated project cost is $59,000.

 

CN1909-032  Select Specialty Hospital - Memphis, Inc.

     For the relocation of Select Specialty Hospital - Memphis, Inc., a 39 Long-Term Acute Care Hospital, from St. Francis Hospital - Memphis located at 5959 Park Avenue, 12 Floor, Memphis (Shelby County), approximately 12 miles northeast to Methodist University Hospital's 10th floor located at 1265 Union Avenue, Memphis (Shelby County).  The applicant is wholly owned by Select Medical Corporation.  The estimated project cost is $17,038,818.

     For the acquisition and placing into service of one additional magnetic resonance imaging (MRI), a 3.0 Tesla MRI at Cleveland Imaging, 2253 Chambliss Avenue NW, Suite 100, Cleveland (Bradley County).  The applicant is owned by Plaza Radiology, LLC.  The estimated project cost is $2,228,675.

 

CN1909-034  Oral Facial Surgery Center, Inc dba Specialty Surgery Center

     410 42nd Avenue North

     Nashville (Davidson County)

     For the relocation of a multi-speicalty ambulatory surgical treatment center (ASTC) from 322 22nd Avenue North, Nashville (Davidson County) 2 miles west to 410 42nd Avenue North, Nashville (Davidson County).  The ASTC has 3 operating rooms (OR's) at its current locaiton and plans to expand to 4 OR's and 3 procedure rooms (PR's) at the proposed location.  The applicant is an "S" corporation owned by Jeffrey B. Carter, M.D., D.M.D.  At some point after licensure, it is planned that The Surgical Clinic, PLLC will take ownership and change the business structure to a limited liability company.  The estimated project cost is $10,279,970.

 

CN1909-035  TriStar Skyline Medical Center

     For the initiation of mobile positron emission tomography (PET) services up to 2 days a week at the campus of TriStar Skyline Medical Center located at 3441 Dickerson Pike, Nashville (Davidson County).  The applicant is owned by HCA Healthcare, Inc.  The estimated project cost is $610,000.

 

CN1909-036  Outpatient Diagnostic Center at Johnson City Center

     3019 People Street, Suite 300

     For the establishment of an outpatient diagnositc center (ODC) and the initiation of magnetic resonance imaging (MRI) services at 3019 People Street, Suite 300, Johscon City (Washington County).  The applicant is owned by Holston Medical Group, PC.  The estimated project cost is $11,005,404.

 

CN1909-037  Cherokee Farm Orthopedic Surgery Center, LLC

     Intersection of Osprey Vista Way and Accelerator Way

     Knoxville (Knox County)

     For the establishment of an ambulatory surgical treatment center (ASTC) located at the unaddressed lot on the University of Tennessee Cherokee Farm Campus east of the intersection of Osprey Way and Accelerator Way, Knoxville (Knox County).  The ASTC will include 5 operating rooms (OR's) with 2 additional shelled OR's for future use.  The 5 OR's subject to this application will primarily be used for, but not limited to, orthopedic surgical cases performed by OrthoTennessee, PC dba University Orthopedic Surgeons.  The applicant is owned by OrthoTennessee, PC.  If the CON is approved, OrthoTennesee is expected to hold 75% of the ownership interest, and Universtiy Health Systems dba UT Medical Center is expected to hold the remaining 25% ownership interest.  The estimated project cost is $19,829,307.

     For the establishment of a 30 bed nursing home at 1513 Main Street, White Pine (Jefferson County).  These beds are subject to the 2019-2020 125 nursing home bed pool.  The applicant is owned by Jefferson County Nursing Home, which is a component of Jefferson County, which is a governmental entity.  The estimated project cost is $8,561,534.

 

CN1909-039  St. Thomas West Hospital

     4220 Harding Pike

     Nashville (Davidson County)

     For the initiation of Cardiac PET/CT services at St. Thomas West Hospital, 4220 Harding Pike, Nashville (Davidson County).  The applicant is ultimately owned by Ascension Health Alliance.  The estimated project cost is $1,640,673.

     

 

Upon written request by interested parties, the staff of the Tennessee Health Services and Development Agency shall conduct a public hearing.

Pursuant to T.C.A. Section 68-11-1609(g)(1), any health care institution wishing to oppose a Certificate of Need must file a written objection with the Tennessee Health Services and Development Agency and serve a copy on the contact person for the applicant no later than fifteen (15) days before the agency meeting at which the application is originally scheduled for consideration.
