Take Charge of Your Diabetes
A Six-Week Workshop
Take Charge of Your Diabetes (the Diabetes Self-Management Program or DSMP) is a six-week workshop that provides tools for living a healthy life for people with diabetes. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that diabetes may bring about.
Self-Management programs supported by the Tennessee Department of Health and University of Tennessee-Extension include the Living Well with Chronic Conditions workshop for people with physical or mental conditions, the Take Charge of Your Diabetes program, and Cancer: Thriving and Surviving workshop for those affected by cancer.
Participants
Take Charge of Your Diabetes workshops are now available in many counties in Tennessee.
Get Involved
There are more than 300 Self-Management Leaders in Tennessee and more than 10 Master Trainers.
All Leaders and Master Trainers must complete a full 4-day training to become certified as a Leader.
Participants must attend the full training and facilitate a program to become a Leader.
Two trained Leaders are required to lead all self-management programs.
