Records for Court Cases
If the records you seek are to be used for a court case, the Department must have a subpoena from the court. Subpoenas may be submitted to the address listed above.

Child Protective Services, Juvenile Justice or Home County Case File records
Fill out the Records Request form and send it, along with a photo copy of your government issued form of identification to the following address or email address listed above.
If you attended a State run DCS facility and are seeking a copy of your education file or transcript, fill out the Records Request form and send it, along with a photo copy of your government issued form of identification, to the address or email address listed above.
For a copy of your GED, you can contact the Department of Labor and Workforce Development, Adult Education Division.
About DCS Records
DCS records are covered by Records Disposition Authorizations, which determine the length of time a record series is maintained. You may review the details in the retention times in the records retention document.
