Forms & Timeline for Textbook Adoption Coordinators

Contact information for district textbook coordinator(s) and anyone who should receive information regarding the adoption subject area(s) should be submitted by August 31 of each school year by all coordinators. A submission form will be posted on this page when available.

The following forms are required to be submitted to the Department of Education by June 15, 2020. These forms should be downloaded, completed, and submitted digitally via this survey:

Additionally, the following form is required to be completed and kept on file in the office of the local directors of schools for the six years of the adoption. This form is required to be completed by October 15, 2020. The department may request this form at any time.
