Welcome to the Records Management Web page. The Records Management Office is here to help you locate records for children that were, or still are, in DCS custody.
There are many different types of records, such as education, medical, adoption, juvenile justice, and others, all obtained by different methods. So, first, determine what type of record you want to find.
Important Note: If the records you are seeking are to be used for a court case of any type, we must have a subpoena from the court. Subpoenas may be submitted to:

Department of Children’s Services, Access to Adoption Records.
Fill out the Records Request form and send it, along with a photo copy of your government issued form of identification to the following address: Department of Children’s Services Records Management Office Sills 3 1286 Foster Avenue Nashville, TN 37210 or fax it to 615-253-5975 or scan it and submit it by e-mail.
Medical files are only kept for a specific amount of time after a child is no longer in DCS Custody, i.e.,until the child reaches age 29, and then destroyed. This is in accordance with Federal and State guidelines If the child has not reached age 29, fill out the Records Request form and send it, along with a photo copy of your government issued form of identification, to the following address: Department of Children’s Services Records Management Office Sills 3 1286 Foster Avenue Nashville, TN 37210 or fax it to 615-253-5975 or scan it and submit by e-mail.
If you attended a State run DCS facility and are seeking a copy of your education file or transcript, fill out the Records Request form and send it, along with a photo copy of your government issued form of identification, to the Department of Children’s Services Records Management Office Sills 3 1286 Foster Avenue Nashville, TN 37210 or fax it to 615-253-5975 or scan it and submit it by e-mail.
For a copy of your GED, you can contact the Department of Labor and Workforce Development, Adult Education Division via their website or you can fill out their GED request form and submit it.
To obtain a duplicate diploma, fill out the Duplicate Diploma Request Form and send it to the address listed on the form.
