RX Outreach
The safety net, for those individuals who have been disenrolled from TennCare because of reform to the program, includes automatic access to Rx Outreach. But, you may be eligible for Rx Outreach even if you are still on TennCare.
The Rx Outreach program is available to individuals and families with incomes of up to 250 percent of the federal poverty level, with these important distinctions:
- If you are still on TennCare, you are not automatically enrolled in Rx Outreach. To enroll, you must complete an application. You may fill out the application form online and print it or print a blank form and fill in your information. The form is available at: http://www.rxoutreach.com/en/dpa1.aspx
- If you are still on TennCare, the generic drugs you obtain from Rx Outreach are not free. You must pay $18 for a 90-day (3-month) supply or $30 for a 180-day (6-month) supply of each medication. Payment information is included on the application.
Your completed, signed application must be accompanied by payment and the written prescription(s) from your prescriber for a 90-day or 180-day supply of drugs on the Rx Outreach drug list.
