How to Apply

Follow these steps.

 

1. Department of Human Services (DHS)

 

Eligibility for TennCare and other Medicaid programs is decided by the Department of Human Services. All 95 counties have a DHS office.

 

  • Find out the address and phone number of your local DHS office.

 

2. Getting an Application

 

You don’t need an appointment to get an application.

 

There are 3 ways to get an application. You can:

 

  • Call and have one sent to you.
    OR
  • Pick up one at your local office.
    OR
  • Print a TennCare application (pdf, 189kb) (spanish) (pdf, 194kb).

3. Giving the Application to DHS

 

You don’t need an appointment to turn your application in to DHS.

 

  • After you complete the application, you can mail it, drop it off, or fax it to your local DHS office. If you need help filling out your application, DHS can help you. Call your local DHS office and tell them you need help with the application.

  • Remember, you can also apply on-line.

 

Once DHS has your application, they’ll tell you if they need any other information from you.
