TennCare offers a broad array of long-term care services designed to help meet your unique needs. Whether you're chronically ill or have a physical and/or intellectual disability, TennCare is committed to meeting the needs of Tennessee residents.

CHOICES

 

Tenncare's program for long-term care services for elderly (65 years of age and older) or disabled (21 years of age and older) including nursing home care or home/community based care.

 

MFP

 

Money Follows the Person (MFP) is a federally funded grant awarded to TennCare intended to assist states in building infrastructures that will result in effective and enduring improvements in community-based long-term care and support systems for people with disabilities.

 

Services for Intellectually Disabled Individuals

 

Provides services for intellectually disabled individuals through home/community based services in three waiver programs or in intermediate care facilities (ICF/MR).

 

PACE Program

 

PACE is a community based adult day care program whose purpose is to serve the frail elderly residents of Chattanooga and Hamilton County.
