Public Records Requests

To make a public records request, contact the Legal Division at (615) 741-2941 or by email. The Department seeks to make public records available for inspection as promptly as possible, but if it is not practicable to make the records promptly available, the Department has seven (7) business days to respond to a request or advise when the records can reasonably be made available for inspection.  

Requests for copies of records must be made in writing, and fees may apply.  Records Request Form (pdf)
