The Division of Business Services is a filing agency and files documents, provides copies and/or certifications of documents.  The Division does not have any legal authority to intervene in disputes between customers, businesses, shareholders, members, officers or other persons. We cannot provide legal advice.

Annual Report Forms

You can now file your business’ Annual Report online by clicking here. After filing electronically, you can view verification of your filing by searching on your company name through the business information search.

If you prefer to file in the traditional paper format, it must be on the preprinted form provided by the Division of Business Services. Forms are prepared and mailed to each active company on an annual basis. Requests for replacement Annual Reports may be submitted to the Division by email, telephone, in writing.
