Complete the Vehicle Information Request (PDF) provided by the Taxpayer and Vehicle Services Division at 44 Vantage Way, Suite 160, Nashville and send the required fees for the information you are requesting. Send the form and a check or money order to:

When filing in person, you must present a copy of your driver license or photo identification. All photocopies of motor vehicle records are certified by this division.
