EXPEDITED REFERRALS TO THE MEU |

The DHS caseworker should note on the MEU
DATA SHEET “EXPEDITED STATUS” for the following: ·
Metastatic
cancer - include hospital admission and discharge notes, pathology (biopsy)
report, operative notes, etc. ·
Other
malignant diseases such as, Hodgkin’s disease, lymphoma, leukemia, melanoma,
and sarcoma, etc. Include the same
medical data as listed above. ·
AIDS/HIV+ -
include current medical data verifying acute and chronic symptoms such as
diarrhea, weight loss, recurrent infections, fatigue, etc. ·
Chemotherapy
– include current medical data to verify severe side effects such as nausea,
vomiting, diarrhea, weight loss, recurrent infections, etc. ·
Severe head
injury - include hospital data. ·
Severe
spinal cord injury, quadriplegic, paraplegic - include hospital data. ·
Severe renal
(kidney) failure and/or hemodialysis - include
hospital records. ·
Severe
coronary artery disease - include hospital data verifying heart attack,
myocardial infarction, strokes. ·
Severe
emphysema/COPD (chronic obstructive pulmonary disease) - include hospital
data. ·
Acute (not
chronic) leukemia - include pathology report to verify diagnosis. ·
Immediate
threats of suicide - include current medical data from the hospital or mental
health provider to verify. ·
Active TB. |
