Survey Instruments - 2012

Quality Assurance survey instruments are revised annually prior to the beginning of each calendar year. The revisions reflect any changes in requirements that have been made during the previous year. Provider manual or other policy changes that occur between revisions in the survey instruments will be considered by Quality Assurance surveyors as they conduct agency surveys throughout the year.
